Team Leader Extra Care

Valley Care has an exciting opportunity for Care Team Leader in the Stockport area to join our new extra care scheme as a Care Team Leader based in the Cheadle area of Stockport, working full time Monday to Friday supporting adults aged 55 and over.

Key Accountabilities:

  • Writing person centred care plans
  • Carrying out new client assessments and risk assessments
  • Undertaking Mental Capacity Assessments and completing Best Interest Decision paperwork
  • Leading on client reviews and undertaking any resulting actions
  • Medication reviews and audits
  • Monitoring the quality of care delivered through spot checks in the field
  • Delivering care when required

Person Profile:

  • Good knowledge and understanding of working with Local Authority Contracts and CQC legislation
  • Proficient IT user
  • Exceptional Time management and ability to prioritise and organise workload
  • Exemplary customer service
  • Excellent Communication skills
  • Ability to work collaboratively as part of a team
  • High attention to detail
  • Passion for high quality and care standards
  • Computer literacy to include Microsoft Word and our own roster system (training provided)

  • Have a right to work within the UK
