We’re recruiting for an experienced Care Assessor to support our home Merton care service.
Our Care Assessors are excellent organisers, communicators, mentors, motivators, problem solvers and team players. They’re the glue that holds our services together. Do you have what it takes to join their ranks?
- This is a full time position Assessor 24 hours (9am - 5pm 3x days per week), caring 16 hours (must be available from 7am)
- Participating in the service’s on-call rota is a requirement for the role
Duties will include:
- Writing person centred care plans
- Carrying out new client assessments and risk assessments
- Undertaking Mental Capacity Assessments and completing Best Interest Decision paperwork
- Leading on client reviews and undertaking any resulting actions
- Medication reviews and audits
- Monitoring the quality of care delivered through spot checks in the field
- Delivering care calls when required
- Partake in the branch on call rota
- Other office support duties as reasonably required

Please note, applicants must have a minimum of one years previous experience working as a field care supervisor or similar role
